Low Country Family Services can provide a home caregiver to help anyone who wants to remain independent but receive assistance due to any illnesses, disabilities, or the natural aging process.

Medicaid Targeted Case Management (MTCM)

Our MTCM services provide Medicaid eligible individuals assistance with accessing medical care, social services, employment, and other resources.
